Physicians desiring to make a referral to the WakeMed Children's Endocrinology & Diabetes Center, pediatric nutrition services or ENERGIZE!, please see below for the appropriate referal forms.
Children's Endocrinology & Diabetes
The WakeMed Children's Endocrinology & Diabetes program provides consultation and accepts referrals for the care for children with endocrine problems including:
- Growth Disorders
- Disorders of Pubertal Development
- Pituitary Disorders
- Thyroid Disorders
- Adrenal Disorders
- Intersex Disorders
- Menstrual Disorders
- Polycystic Ovarian Syndrome
- Disorders of Calcium and Phosphorus Metabolism
- Disorders of Carbohydrate Metabolism Including Hypoglycemia
- Diabetes including Type 1, Type 2, Neonatal, and MODY
- Turner Syndrome
- Prader-Willi Syndrome
- Klinefelter’s Syndrome
- McCune-Albright Syndrome
- Lipid Abnormalities
- Russell Silver

Emergency On-Call Process
If you see a child in your office with new onset diabetes mellitus or are in need of an immediate endocrine consult, call WakeMed's 'Provider to Provider' service at 919-350-4P2P (350-4727) to be connected to the Children's Endocrinology & Diabetes provider on call. Our provider will decide whether your patient needs hospitalization or can be seen in our outpatient office.
If your call is not urgent, please contact the Children's Endocrinology & Diabetes office at 919-350-7584. We are open Monday through Friday, 8 am to 4:30 pm.

Outpatient Pediatric Nutrition Services
WakeMed Cary Hospital outpatient nutrition services features registered dietitians who specialize in pediatric nutrition. Specifically, they help patients and their families with nutrition plans for the following:
- Failure to Thrive
- Growth Disorders
- Celiac Disease
- Food Allergies
- Weight Management
- Other Nutrition Issues

WakeMed ENERGIZE! Physician Referral
Designed for children ages 6-18, ENERGIZE! consists of a family-centered approach to making healthy lifestyle changes. Patients will receive an initial health behavior evaluation as well as ongoing education and support. Based on need and motivation levels, each family will be enrolled in one of the following options:
- A community-based, 12-week exercise and educational program that meets three times a week and includes group healthy lifestyle education and fitness programs
- Individual medical nutrition therapy and healthy lifestyle education sessions, including individual meal and fitness plans
