Prior Authorization Requirements at 210 PET Imaging
Patient’s whose insurance is in-network with 210 PET Imaging and with prior authorization on file can be scheduled. Please attach a copy of the authorization with your order.
Self-pay patients can be scheduled by making a 25% deposit on the discounted exam price at the time of scheduling. The self-pay patient will pay the remaining 75% of the discounted exam price on the day of the exam. If patient is unable to pay remaining 75% at date of service, the appointment will be cancelled.
If any of the below are inaccurate at the time Financial Clearance reviews the account, it will potentially result in a denial; therefore, a payment agreement or rescheduling of the patient will result. All authorization issues must be resolved within 48 hours (about 2 days) or shorter prior to the appointment and depending on tracer type ordered
