Health Information Management (HIM)

Patient Requests for Information

If you are 18 years of age or older, and mentally competent, you will request your health information.

You may request health information for another individual, but you may be asked to provide documentation that supports your authority to access the information before the request can be processed.

You will need to submit your requests in writing to the HIM Department. To make a request in writing, fill out our Authorization to Release Medical Information form:

HIM can provide you with an abstract of your health information. An abstract can include any of the following as applicable:

Abstract of Information 

Office Visit

ED Visit

Hospital Stay

Demographics (includes name, address, last 4 of SSN)

 

 

History & Physical (tells why you were admitted to the hospital)

 

 

Consults (tells why you were seen by a specialist while in the hospital)

 

 

Discharge Summary (outlines the treatment you received during your hospital stay and includes any lab work/imaging studies that were made during your stay)

 

 

Progress Notes (a note made by a provider that tells why you were seen in the office and the treatment you were prescribed includes labs/imaging)

 

 

ED Visit (tells why you were seen in the ED, outlines the treatment you received while you were in the ED along with lab work/imaging studies that were made during your visit to the ED

 

 

 

Requests for information other than an abstract, or requests for dates of service prior to February 1, 2015, are sent to our copy service to be reproduced. The copy service will charge a fee as outlined:

Electronic Copies (CD or Portal) 6.50 Flat Rate
Paper Copies 0.07 per Page
Mailing CD or Paper Additional charge for postage will be added

Attorneys representing patients for personal injury or social security claims are billed at the State rate with additional fee for postage added as applicable.

Once received, your request will be processed as quickly as possible, no later than 30 days following receipt, unless you are notified of the need for a one-time, 30-day extension.

At WakeMed, our HIM Department has Release of Information Specialists available Monday through Friday 8:30 am to 5 pm at our Raleigh and Cary hospital locations to help you access your health information.
