Volunteer Forms and Information

All new (approved) and current volunteers are required to complete specific forms, such as New and Annual Education; Occupational Health Medical Screening Confidentiality Agreements; and others mandatory requirements, to begin active service and remain current in their roles.

Select either the new or current volunteer button below to find information, forms and trainings. A link to the volunteer handbook is also available. This handbook is a first-stop resource for important information related to your service at WakeMed.

If you have any questions, please contact Cary Volunteer Services at volunteerservicescaryhospital@wakemed.org or Raleigh/North Volunteer Services at volunteerservicesraleighcampus@wakemed.org

We appreciate your interest in serving our patients and staff.
