Clinical Case Management
In the acute care hospitals, the case manager is a social worker or registered nurse who works directly with you during your acute hospital stay. The acute hospital case manager will help you find the right level of care, support and services you need as you leave the hospital and continue your journey. In the rehab hospital, each patient has his or her own WakeMed Rehab case manager to provide support, information and referrals to hospital and community services upon completion of your inpatient rehab stay. All of our rehab hospital case managers are masters-prepared and who work directly with patients and their family members and can assist with:
- Discharge planning needs after you no longer need hospitalization
- Developing your individualized plan of care and treatment while a patient in the rehab hospital
- Understanding and navigating the financial impact and the health insurance coverage process
- Setting individualized treatment goals and desired outcomes for your inpatient rehab stay
- Preparing you and your family for your return home and in the community
