Download Referral Form (PDF)Our Diabetes Education Program offers comprehensive classes to help individuals manage their diabetes effectively. We provide both in-person and virtual classes led by Certified Diabetes Care and Education Specialists and Registered Dietitians.
For Scheduling: A physician referral is required for scheduling. Download a referral form or call 919-350-7292 to request a form. Referrals can be sent via Epic or paper referral fax to 919-350-7400
In-Person Diabetes Education Classes
The class topics include understanding diabetes, healthy eating, diabetes medication, and exercise & stress management. In-person classes will provide a voucher for lunch.

Medicare coverage of DSMES requires a provider referral. The treating qualified provider must maintain document
- A1C > 6.5%
- Fasting blood glucose greater than or equal to 126 mg/dL on two different occasions
- 2-hour post-glucose challenge greater than or equal to 200 mg/dL on two different occasions
- Random glucose test over 200 mg/dL for a person with symptoms of uncontrolled diabetes
Virtual Diabetes Education Classes
Class One - Diabetes Basics
First Thursday of each month
1:30 to 3:30 pm
This class will cover types of diabetes: Type 1, Type 2 and gestational diabetes, diagnostic criteria, acute complications (hyperglycemia, hypoglycemia), medications to control diabetes (orals, insulins and other injectables), standards of care and foot care.
Class Three – Diabetes Self-Management
Third Thursday of each month
1:30 to 3:30 pm
1:30 to 3:30 pm This class is designed to prioritize understanding of daily glucose records and techniques on how to properly use insulin and blood glucose monitoring technologies. Strategies for sick days and stress management will also be covered.
Class Four – Healthy Eating Lifestyle Changes
Fourth Thursday of each month
1:30 to 3:30 pm
1:30 to 3:30 pm This session covers long term sustainable changes for diet and overall health. Topics include making smart food choices, meal planning and grocery shopping. The class will also cover lifestyle habits including eating habits, hydration, exercise and stress management.
