These organizations offer behavioral health services on an outpatient basis. Patients receiving outpatient care receive services without being admitted to the hospital. Here are the benefits:
- Available clinical and medical information about a referred patient prior to appointment including an assessment of level of acuity/risk.
- An educated and oriented patient who has been briefed on illness trajectory treatment options.
- Pre-qualified referrals which match NABH provider specialties and locations to patient need.
- Advanced access to social service providers participating in the Connected Community.
- In certain scenarios, direct admission to BH crisis and inpatient facilities.
- Shared care coordination services including data aggregation/normalization, reporting, analytics, transitional care services and IT, patient engagement tools, quality improvement initiatives, network management and technical assistance.
- Population health advocacy, innovation, and education materials and training.
- Value Based Contracting incentives related to health outcomes achieved at a population and/or community level (*the NABH does not negotiate on behalf of or represent member on fee for service contracts).
