Thank you for choosing WakeMed Physician Practices - Urology. We want your visit with us to be pleasant, so please take a moment to review what to bring with you, how to find us, our list of doctors, and office hours.
Our Pledge to Patients
- We pledge to provide you with personalized, sensitive care.
- We will always keep you informed of your diagnosis, and you will play an active role in the decision-making process.
- We pledge to provide you with the latest advances in urology.
- We pledge to respect your confidentiality as a patient.
- We pledge to provide you with the most cost-efficient healthcare. In an effort to curb costs we will recommend tests and procedures only when appropriate, and we encourage you to obtain second opinions if you so desire.
- Above all, we pledge never to compromise on our first priority: the well-being of our patients.
For your convenience and timeliness, we request that you bring the following when you come for your office visit:
- Bring your insurance information.
- Please arrive 15 minutes before your scheduled appointment to ensure that we have complete, updated information on you.
- Your co-pay amount that will be requested at check-in time (we accept cash, debit cards, Mastercard and Visa).
- Your driver's license or another form of identification
- A list of your medications and allergies (along with your pharmacy name, address and phone number).
- The patient information form and medical history form. This is information that your Doctor will need in order to treat you properly.
- All relevant medical records, lab results, x-rays from your Primary Care Doctor (without this information our Doctor will not be able to evaluate you properly so we will have to reschedule your appointment). Men 40 + years should be sure to bring PSA lab results from their primary care doctor.
- Your Primary Care Doctor's name, address and phone number.
- Authorization number from your Primary Care Doctor's office faxed prior to your visit (if your insurance company requires an authorization.) Without the required authorization, you may be asked to sign a waiver and pay for your visit or reschedule your appointment.
- If you are late for your appointment, you may need to reschedule if we are unable to work you into that day's schedule.
