School Information
Following your child's diabetes diagnosis, you must contact your child's school-based public health nurse. The nurse will arrange for your child's care in the school setting which includes reviewing the diabetes care plan, ensuring the training of appropriate school staff, and discussing your child's diabetes supply needs for school.
At each visit, our office will provide your child with a comprehensive, physician-authorized Diabetes Care Plan to assist the school with your child's diabetes care. If you need an updated form printed between visits, please call (919) 350-7584 to request a form.
Inside Wake County
If your child attends a Wake County public school, please call (919) 250-4631 and ask for your child's school nurse. You will be given the nurse's voice mail number. Leave a message for the nurse to discuss your child's diabetes care at school.
Outside Wake County
If your child attends a school outside of Wake County, contact your child's school directly to obtain the nurse's contact name and phone number.

In an effort to help your child manage his or her carbohydrate intake while at school, you may find it helpful to review the Wake County Public Schools website.
This site provides carbohydrate counts for each item on the Wake County Public Schools child nutrition menu.
